Panaritium dissection

Panaritium is an acute purulent inflammation of the tissues of the finger, which most often occurs as a result of infection due to a microtrauma, splinter or cut. The disease progresses rapidly, causing severe pain, swelling, redness and impaired function of the affected finger. If left untreated, the inflammation can spread to tendons, bones or joints, leading to severe complications. Opening a panaritium is a surgical procedure aimed at evacuating pus, relieving tissue tension and preventing the spread of infection.

When surgical intervention is required

Any purulent panaritium that cannot be treated conservatively is an indication for opening. In particular:

  • the presence of painful throbbing pain in the finger;
  • severe swelling and redness;
  • restriction of phalanx mobility;
  • formation of an abscess under the skin, nail or in deeper tissues;
  • Fever, general malaise.

With a confirmed diagnosis, surgery is the only effective treatment. Otherwise, an abscess, phlegmon or osteomyelitis may form.

Contraindications to the procedure

There are no absolute contraindications to abscess opening, as the intervention is urgent. However, the procedure may be postponed or adapted in the following cases

  • blood coagulation disorders;
  • decompensated chronic diseases;
  • a patient's serious condition that requires stabilisation prior to surgery;
  • allergy to local anaesthetics - in this case, an alternative anaesthetic is selected.

How is the panar section performed?

The procedure is performed on an outpatient basis in a small operating room. Before the intervention, the doctor examines the affected area, assesses the depth of the lesion and the scope of the intervention. The main stages:

  1. Anaesthesia - local anaesthesia blocks pain in the area of surgery.
  2. Skin incision - a small surgical incision is made in the area of pus accumulation.
  3. Evacuation of contents - pus is removed, after which the wound is thoroughly washed with antiseptic solutions.
  4. Drainage - if necessary, rubber or latex drainage is installed.
  5. Dressing - a sterile dressing is applied, sometimes with an antibacterial agent.

The procedure usually does not exceed 20-30 minutes. The patient can return home immediately with recommendations for wound care.

How to prepare for the procedure

No special preparation is required, but it is advisable:

  • do not apply ointments or alcohol compresses on your own, which can aggravate the process;
  • do not pierce the abscess yourself;
  • inform the doctor about chronic diseases, allergies, anticoagulants or other medications;
  • avoid drinking alcohol the day before the visit.

In case of fever or increasing pain, it is important to visit the clinic as soon as possible - self-medication can be dangerous.

Additional consultations and related services

For a comprehensive approach to treatment, the patient may need additional consultations:

  • Surgeon - performs the procedure and conducts postoperative monitoring;
  • Traumatologist - if the panaritium occurs after a complex trauma;
  • Endocrinologist - for patients with diabetes mellitus who are at high risk of complications;
  • Dermatologist - in case of repeated abscesses or suspected dermatological pathology;
  • Ultrasonography - to determine the depth of the lesion in case of subcutaneous or bony panaritium;
  • Online consultations - for a preliminary examination or monitoring of healing;
  • Check-Up - a general examination of the body in case of frequent infections.

Frequently asked questions about opening a panaritium

1. Is it possible to cure panaritium without opening it?

In the initial stages, it is possible to use heat baths, antiseptics and antibiotics. However, if there is already an accumulation of pus, only surgery is effective. Delaying the operation increases the risk of complications.

2. How do I know when it's time to go to the doctor and not treat it at home?

If the pain is pulsating, the finger is red, hot to the touch and swelling occurs, these are signs of a purulent process. Self-piercing or heating will only aggravate the situation. In this case, you need to consult a surgeon.

3. Does the panaritium opening procedure hurt?

Before the opening, local anaesthesia is always performed, which completely anaesthetises the area of intervention. There may be mild discomfort after the procedure, but the pain decreases significantly immediately after the pus is removed.

4. How long does the finger take to heal after surgery?

Usually, complete healing takes 7-10 days. With proper care and adherence to recommendations, complications are rare. In deep forms (bone or tendon panaritium), recovery may take longer.

5. Can I work with my hands immediately after the opening?

It is recommended to limit the load on the affected hand for at least a few days. It is important to avoid contact with dirt, water and chemicals until it is completely healed. The doctor may issue a temporary sick leave.

6. What to do if panaritium recurs?

In such cases, you need to undergo additional diagnostics. Frequent recurrences may be associated with endocrine disorders, reduced immunity, or untreated infection sites. We recommend a consultation with an endocrinologist, dermatologist, and a comprehensive body examination (Check-Up) at our clinic.
