How to sign a declaration with a family doctor

You can sign the declaration online (by sending your documents) or at a doctor's appointment.  Click on the “Online appointment” button, fill in all the necessary fields and wait for a call from our specialist.

  • After submitting the application, the manager will contact you to confirm and sign the declaration. If you need to change your doctor, the current declaration will be canceled automatically. 

The following documents are required to stow the declaration:  

• passport;

• identification code;

• phone number of a trusted person for emergency communication;

• cell phone number for receiving an SMS code.

In case of signing the declaration with a doctor for a child under 14 years old:  

• Child's birth certificate;

• passport and identification code of parents or guardians;

• phone number of the authorized person for emergency communication;

• cell phone number for receiving an SMS code.

Влітку 2024 року я уклала Декларацію с Панасюком Олексій Віталійовичем, з першого відвідування він пропонував мені пройти мамографічне обстеження молочних залоз. Ніяких непокоєнь не було. Нічого не боліло. Я проігнорувала його рекомендацію, але при кожному наступному відвідуванню він наполягав на такому обстеженні. Лише у грудні 2024 я пройшла це обстеження. Результат: злоякісна пухлина 1 стадії і я готуюся до операції. Дуже сподіваюся, що встигну і не буде гірше. Дуже вдячна своєму сімейному лікарю за наполегливість. Можна сказати він мене врятував. Хочу виразити Панасюку О.В. щиру ПОДЯКУ!!! А всім дівчатам побажати дуже уважно ставитися до свого здоров'я, не боятися мамографа і прислухатися до порад лікаря! Бажаю здоров'я!!!
